Board Director Vacancy

The North Staffordshire GP Federation are seeking two new members to join our Board of Directors.  The Board usually consists of up to 9 Directors, which includes a mix of GPs, Nurses and Practice Managers.  Therefore, we are looking for two people to join as Directors to assist in holding the Executive Directors and Management Team to account and helping to develop our Strategy and vision for the Federation going forward.

 

The minimum commitment of a Director is one session a month with the expectation that you attend the monthly Board meeting, read the papers for the Board in advance and contribute to any discussions via email or potential other forums.  There is the opportunity to be involved in other projects connected to the Federation too but the core requirement is the one session a month.

 

Remuneration is the same for all Directors at £80 per hour (so the one session a month would be £320 for a four-hour session).  The Board Meetings are nearly always 3:30 till 6 on a Thursday and are generally held at Middleport Medical Centre.  The rest of the obligations can be worked flexibly.

 

As an organisation, we are here to support Primary Care and make Primary Care supportive and resilient and steadily evolve to where it needs to be for its future sustainability for itself and for the patients we serve.  This now includes supporting the new Primary Care Networks in Northern Staffordshire, assisting them in areas they require it and where it makes sense to do something once rather than 13 times.

Job Summary

To provide a point of contact for patients and to act as a focal point of communication between patients, doctors and other medical and clerical staff.

 

Main Duties and Responsibilities

  • Greet patients coming into the surgery and direct them appropriately
  • Create and update the appointment book under the supervision of the Reception Manager
  • Ensure that patients requiring appointments are seen in a logical manner according to urgency.
  • Run the surgery appointment system effectively
  • Accurately record baby immunisation clinics onto the appointments screen & liaise with child health in relation to changes to clinics
  • Run clinics effectively
  • Follow up patients who do not attend for their appointments at the surgery
  • Follow up patients who do not attend for their retinal screening appointments
  • Take appropriate details from patients wishing to register with the practice and provide appropriate information about the practice
  • Inform patients of their named GP
  • Explain practice arrangements to patients and those seeking temporary cover.
  • Accept and register patients in accordance with practice procedures
  • Update patient demographic information when changes are identified.
  • Advise patients of relevant charges for private services, accept payments and issue receipts.
  • Respond to queries and request for information from patients and other agencies.
  • Adhere to information governance policies and procedures
  • Take requests for prescriptions accurately and enforce the repeat prescribing policy.
  • Produce acute / repeat prescriptions under the supervision of the Practice Pharmacist
  • Promote on-line access for booking of appointments and requesting of repeat prescriptions
  • Record and book requests for ambulances.
  • Ensure the reception and waiting areas are kept neat and tidy
  • Ensure infection control procedures are maintained with regard to cleaning of the reception and waiting areas (including the cleaning of vomit/faeces in accordance with practice procedures and the regular cleaning of the childrens toy)
  • File and retrieve medical records as required, ensuring strict alphabetical order.
  • Ensure that correspondence (paper and electronic) are directed quickly and correctly.
  • Receive and make telephone calls as required / to accurately record messages.
  • Ensure night service is taken off the telephones each morning and placed in operation at the end of each day.
  • Open the surgery premises and prepare for surgery
  • Secure premises according to practice guidelines.
  • Document all requests for reports to be completed by the GP
  • Complete referrals to the district nurse team under the supervision of the GPs
  • Process the actions recommended by clinicians from hospital letters and pathology/radiology results
  • Process reports received from the out of hours service and advice lines
  • Produce pathology requests as directed by the clinician
  • Complete cytology returns on Exeter and complete smear recall programme
  • Maintain awareness of target patient groups e.g. GSF, ILCT, Admissions Avoidance

Job Summary

The Advanced Nurse Practitioner is responsible for the delivery of the nursing and medical needs of patients with chronic disease through proactive case management of patients conditions including liaison with both internal and external professionals and other agencies to manage the patient effectively. The post will also include telephone triage and initial contact for patients requiring urgent (on the day) treatment) The Advanced Nurse Practitioner is directly responsible to the Senior partner, but works in support of all the Practice GPs and other clinical staff as part of a cohesive clinical team.

 

Main Duties and Responsibilities

  • To write, and maintain policies within the practice for clinical areas and to oversee the development of policies written by the practice nurses
  • To promote best practice in the implementation of such policies
  • Identify, monitor and review deficiencies in procedures and implement remedial action
  • To work with colleagues to ensure contractual targets are met
  • Provide a first point of contact in the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making to establish a diagnosis and management plan
  • Instigate necessary diagnostic tests or investigations and interpret findings/reports
  • Instigate radiology requests ensuring training in accordance with the local hospital trust
  • Monitor indicators of chronic disease anticipating possible decline and proactively manage to enhance well-being and maintain independence
  • Prescribe medication and review medication for therapeutic effectiveness
  • Develop evidence based specialised care plans for patients
  • Plan interventions in light of patients expressed wishes and goals of care

Health Care Assistant – Homeless Service

(£21,350 – £24,000 based on a 37.5 hour working week equivalent, dependent on experience)

Job Summary

North Staffordshire GP Federation is a membership organisation to all 71 GP practices, currently serving the whole of North Staffordshire, including, Stoke-on-Trent, Newcastle under Lyme and Staffordshire Moorlands.  Member practices currently provide services to over 500,000 patients.

North Staffordshire GP Federation currently run the Walk in Service at Hanley Primary Care Access Hub and the local Extended Access to Primary Care service where patients are able to access pre-bookable evening and weekend appointments from 5 hubs across North Staffordshire (Hanley, Longton, Bradwell, Haywood and Leek). 

In addition to this, we have recently been awarded a 2-year Contract to support the range of homeless services available in Stoke-on-Trent.  As a result, we are currently seeking a Health Care Assistant to support the service we will be designing and delivering.

Successful candidates will be competent in being able to deliver the clinical procedures required of the job, be excellent communicators with good IT skills and who are passionate and enthusiastic about providing services to this diverse patient group.

Key duties include:

  • Be expected to foster and maintain positive relationships with homeless individuals with complex needs as a part of a team.
  • Provide a liaison function between the clinical teams providing care for HICNs and their registered GP Practice if they have one
  • Facilitate and support HICNs in using primary care services in a positive and appropriate way.
  • Provide routine physiological measurement, monitoring and support within their competence for HICNS who have, or may have, long-term health issues
  • Undertake review dressing changes and wound care following a care plan
